Delivery System Reform

The Patient Protection and Affordable Care Act appropriates new funding for services that complement and strengthen medical homes, with a particular focus on beneficiaries with chronic needs. This federal support furthers opportunities for states to place primary care at the core of delivery system transformation. CHCS is working with Medicaid programs to design health home models that offer integrated, coordinated systems of care for adults and children with complex needs.
