These Medicaid fact sheets highlight opportunities to improve health care quality in the 15 communities participating in the Robert Wood Johnson Foundation's Aligning Forces for Quality (AF4Q) initiative. Each fact sheet includes a regional Medicaid profile as well as national facts.

Medicaid partners, including health plans, quality improvement organizations, and provider practices, can use these fact sheets to inform collaborative, system-wide quality improvement efforts - both within the AF4Q initiative and beyond.
