This BCAP Pilot Planning Form offers an easy-to-use planning template to complete when initiating new quality improvement initiatives. Completing the form encourages health plans to identify quantifiable, measurable goals for quality improvement projects and to document necessary action steps, key stakeholders, and potential barriers. One form should be completed for each BCAP Typology category: Identification, Stratification, Outreach, and Intervention.
