Nearly half of the U.S. population lives with a chronic condition. The burdens associated with these conditions are high — from the impact on quality of life at the individual level to the costs associated with treating chronic illness. More than 80 percent of health care dollars in this country are spent on those with chronic illnesses.1 Empowering both physicians and patients to effectively manage chronic illness is an important step in improving care quality and reducing costs.
