Care Management Strategies for High-Risk Pregnancy and High-Risk Asthma
OVERVIEW
Many states and health plans are targeting quality improvement efforts for Medicaid’s highest-risk, highest-cost populations both to improve outcomes and achieve a return on their investments. High-risk asthma and high-risk pregnancy are two areas where health plans and state Medicaid agencies that manage care more effectively can reap “quicker” returns, which can then be reinvested to improve quality for other beneficiaries. This CHCS webinar Conference outlined innovative approaches to identify and stratify the risks for children with severe asthma and women with high-risk pregnancies and design targeted interventions to improve outcomes. Leading health plan and state representatives shared provider and consumer-based care management strategies that have resulted in significant reductions in utilization for these high-risk members.
