Over a four-year period, in partnership with the New York State Department of Health and with generous support from the New York State Health Foundation, the Center for Health Care Strategies (CHCS) facilitated the New York Health Homes Learning Collaborative, which included 35 Medicaid Health Home provider organizations from across New York state. The collaborative guided ground-level implementation efforts, shared emerging best practices, and informed state policy decisions around how to best serve the state’s most vulnerable individuals.

CHCS asked participating health home organizations to provide “success stories” that chronicled how the project was impacting individuals. This compendium provides a sampling of those stories, and serves as a testament to the incredible efforts of the people involved in this project.
