With the highest number of people experiencing homelessness in the country, California health care leaders and policymakers are seeking new ways to improve health outcomes for this population. Illumination Foundation, a nonprofit organization in southern California, created an integrated care model that offers recuperative care, emergency shelter, temporary and permanent housing, and primary care for clients experiencing homelessness with complex needs. Their care model uses risk profiles and health-related social needs data to improve care for this population.
