By synchronizing asthma interventions at the health plan and provider practice level, organizations can potentially accelerate the benefits of quality improvement interventions. This two-part tool uses the Chronic Care Model¹ to outline provider site interventions and complementary health plan activities to improve asthma care. Organizations can use the tool to guide the joint development of plan/provider quality initiatives to improve asthma management.

The provider site improvement guide was developed by the National Institute for Children’s Health Care Quality. The list of plan activities to support provider site improvements was originally developed by the New York State Department of Health and modified, with permission, by the Center for Health Care Strategies.
