Community health workers, promotores, and community health representatives (CHW/P/Rs) play a growing role in helping people navigate health and social services. Central to their work is building trusted relationships with the communities they serve, often grounded in shared lived experiences.
As Medicaid programs, including California’s Medi-Cal program, expand reimbursement for CHW/P/R services, managed care plans and community-based providers need tools to support implementation and long-term sustainability of these services.
This resource center, developed by the Center for Health Care Strategies in partnership with the California Health Care Foundation, features practical guidance and tools for managed care plans, providers, and community-based organizations interested in building and sustaining CHW/P/R programs.
