Changing how care is delivered is challenging, particularly for safety-net providers and others serving a high proportion of Medicaid members. CHCS helps states develop programs that will fundamentally improve how care is delivered to Medicaid populations. We help states build coordinated and integrated care models that work with relevant behavioral, supportive, and public health services to address broader determinants of health and prevent patients from developing chronic conditions.
