California is entering a period of major change in Medi-Cal (Medicaid) policy. Federal H.R. 1 and related state policy changes will affect members’ eligibility and coverage, with many related impacts occurring at the local level. Counties, Medi-Cal managed care plans, providers, community-based organizations, and other safety-net partners will play an important role in supporting continuity of coverage, strengthening communication with members and health care partners, and responding to the needs of people who experience changes in coverage. Many regions are already planning how to respond, but capacity and infrastructure vary across counties and regions in California.
To help communities prepare, the California Health Care Foundation, The California Wellness Foundation, and Kaiser Permanente are supporting Medi-Cal Regional Exchange for Access, Coverage, and Health (Medi-Cal REACH), an 18-month learning community bringing together 20 county- and regional-level partnerships across California (see list below). The initiative includes grants to support cross-sector coordination, collaboration, and convenings in response to H.R. 1 and related Medi-Cal policy changes that could increase coverage losses and demand for locally funded health care and safety-net services.
The Center for Health Care Strategies (CHCS) serves as the Program Office for Medi-Cal REACH. Through the initiative, CHCS will support local leaders as they develop aligned, cross-sector approaches to prevent avoidable coverage loss and strengthen coordination. CHCS will also distill and share lessons from participating communities to inform similar efforts across California and nationally.
Learn More and Connect with Regional Partners
Organizations serving the participating counties listed below can contact CHCS to learn about local Medi-Cal REACH activities. Through this form, organizations can ask questions or request a connection with the appropriate regional lead entity for their county. Organizations elsewhere in California may also use the form for general questions about the initiative.
