This webinar, hosted by the Center for Health Care Strategies (CHCS) with support from the California Health Care Foundation, is the fourth in an Assisted Living for Medi-Cal Enrollees learning series. The series is exploring opportunities to strengthen cross-sector partnerships to improve appropriate, timely, and effective use of assisted living communities for Medi-Cal enrollees, including older adults, people with disabilities, and individuals with behavioral health needs.

This webinar will focus on barriers experienced by Medi-Cal members in maintaining their residence in assisted living communities and strategies to address them. It will explore key topics, including supporting residents with significant behavioral health needs and/or who face challenges paying for room and board. The session will also highlight roles of different stakeholders during service provision at one assisted living community.

Managed care plan staff, Enhanced Care Management and Community Supports providers, assisted living waiver care coordination agencies, county behavioral health and mental health plan providers, assisted living operators and providers, hospital and skilled nursing facility providers, and social services providers, among other interested stakeholders, are encouraged to attend this 75-minute event.

I. Welcome and Overview

Speaker: Zackiya Grant-Knight, Program Officer, CHCS

Z. Grant-Knight will welcome participants and provide an overview of the Assisted Living for Medi-Cal Enrollees: Virtual Learning Series. She will review common services provided within assisted living communities for select Medi-Cal populations and highlight challenges faced by stakeholders in providing wraparound services. Additionally, she will provide an overview of financial options and resources available to support services for assisted living residents.

II. Service Provision in Assisted Living

Speakers: Angela Kutnerian, Wellpointe; Schalon Woods, Alameda County Health; Bo Buena, Kern Health Systems

Speakers will share strategies to support residents in assisted living communities related to wraparound services and costs. A. Kutnerian will present on Wellpointe’s role in coordinating services to ensure that residents have the necessary supports to live successfully in assisted living. S. Woods will highlight resources available in Alameda County for people with behavioral health needs in assisted living communities. And B. Buena will share how Kern Health Systems supports members so they can afford assisted living costs, including room and board payment.
