Unite Us is a technology company that builds coordinated care networks of health and social service providers — enabling partners to use data-driven insights to improve care delivery. The Unite Us platform supports collaboration, community-wide care coordination, and secure, bidirectional data sharing. Local Unite Us team members support comprehensive network-building and a change management process that includes onboarding, training, growth, and ongoing support.

Results

With Unite Us, community networks have achieved: 96 percent accuracy of electronic referrals, 88 percent improvement in coordination efficiency, and 84 percent of patients receiving specific resolved health and social outcomes delivered by external partners. The technology has enabled network partners to remove significant administrative costs, and reduce the time to care for both health care and social services. Network stakeholders and community providers utilize outcome data to measure impact while addressing gaps in respective communities.
