Product Description
CipherHealth’s patient engagement suite — including telephonic, text, e-mail, and app-based programs — enables providers to engage low-income and complex populations before, during, or after care episodes. CipherHealth’s platform proactively educates patients, identifies issues or concerns, and alerts designated staff members to resolve identified issues. Trend data are collected to identify areas for short- and long-term improvements. The platform also helps health care providers connect with community resources to address patient issues directly.
Results
CipherHealth’s platform has helped providers achieve the following results: (1) a near eight-fold increase in the number of completed cancer screenings in a study of 250 California PRIME eligible patients over six months; (2) a 60 percent decrease in HIV readmissions in a study of an academic medical center with 281 patients over nine months; and (3) a 35 percent increase in flu vaccinations in a study of 26,200 DSRIP eligible patients over one year. Additionally, the platform achieved a greater than 50 percent engagement rate in an analysis of more than one million Medicaid and Medicare beneficiaries that led to reduced readmissions, decreased emergency department utilization, and improved patient experience scores.
