LivWell Health’s Navigate platform enables care teams to: (a) screen for social determinants risk; (b) generate a patient-centric service plan; (c) pre-authorize services; and (d) better-manage service referrals to home and community-based services. It offers enterprise-class features, including single sign-on, HIPAA-compliant security standards and API. Among the populations the platform is used with are patients that are members of a bundled payment (e.g., joint replacement), Medicare Advantage, Medicaid waiver, PACE or ACO program, high risk (elderly, frail) patients discharged from the hospital into the home, and health systems seeking to reduce readmissions, improve length-of-stay, patient satisfaction and operational efficiency.
Based on customer data, LivWell Health’s secure, web-based solution has been proven to improve staff efficiency by 50 percent and improve clinical outcomes (50 percent reduction in falls; 71 percent improvement in falls risk scores; 82 percent of patients felt more confident).
