Product Description

Certintell is a telehealth platform that goes beyond telephonic calls to help improve engagement and understanding of patients’ wellbeing. Through HIPAA-compliant video and messaging, Certintell empowers community health centers to start or scale telehealth services for Medicare and Medicaid populations – with the ability to schedule visits directly from the platform. These services are integrated into current health center workflows to streamline everything from annual wellness visits to chronic care management and beyond. Certintell optimizes care by supplementing community health centers with remote health coaches to deliver value-based care — improving outcomes, quality, and costs.

Results

Certintell partnered with Federally Qualified Health Centers (FQHCs) as part of an Access Increases in Mental Health and Substance Abuse Services (AIMS) grant. Early results showed increased education and collaboration for primary care providers with hepatologist teams, increased access to psychiatry services via telehealth, and improved satisfaction with patients being able to see mental and behavioral health providers on the same day. Behavioral health providers also shared early positive results of decreased travel time and increased number of patient visits. In 2019 Certintell began a trial with three North Carolina FQHCs, implementing a remote patient monitoring pilot. Results will be posted at a future time at Certintell.com.
