With more than 181,000 people living on the streets and in shelters, California is home to more people experiencing homelessness than any other state in the nation. California’s CalAIM initiative has created new opportunities for homeless service organizations to partner with the health care sector to align services and enhance care delivery for individuals experiencing or at risk of homelessness.

The profiles share insights from pilot teams that participated in Partnerships for Action: California Health Care & Homelessness Learning Collaborative, an initiative supported by the California Health Care Foundation and led by the Center for Health Care Strategies.
