The Medicaid Health Home State Plan Option allows states to create Medicaid health homes to coordinate the full range of medical, behavioral health, and long-term services and supports needed by beneficiaries with chronic conditions. To help states develop these new models, the Centers for Medicare & Medicaid Services (CMS) established the Health Home Information Resource Center.
Coordinated by Mathematica and the Center for Health Care Strategies, the resource center offers publications to help guide health home development and implementation. One-on-one technical assistance and peer-learning opportunities are also available.To help states plan their health home programs, the resource center is regularly updated with new resources from CMS, select external organizations, and states.
