California faces a pressing challenge in addressing the health and social needs of people experiencing homelessness. Through CalAIM, the state’s Medicaid transformation initiative, managed care plans and local continuums of care are partnering to achieve a more coordinated, person-centered, and equitable system of care for all Californians, including those experiencing homelessness.

In California’s Central Valley, a cross-sector partnership is finding new ways to improve care for individuals experiencing homelessness in Kings and Tulare Counties. The Kings Tulare Homeless Alliance, alongside managed care plans Anthem Blue Cross, Health Net, and CalViva Health, is integrating new CalAIM services into the local homeless response system, including through the Kings/Tulare Coordinated Entry System (CES).

This CES navigation map can help health care and homeless services providers — such as medical respite care and street medicine teams — better understand the CES and how to help individuals connect to housing and health care services. While tailored to the Kings-Tulare context, the tool can help inform other partnerships in California and across the U.S., seeking best practices for coordinating systems and services for people experiencing homelessness.
