Innovative strategies are needed to successfully address the health-related social needs of Medicaid members. As health disparities have remained unchanged over decades, solutions must overcome social, economic, and environmental disadvantages experienced by enrollees, with the goal of expanding access to services and improving health outcomes. The Medicaid Innovation Collaborative (MIC) convenes states, health plans, and entrepreneurs to pilot technology solutions that address beneficiary social needs related to food security, housing affordability and quality, and transportation security.
These case studies are part of a series featuring partnerships across managed care organizations and technology companies that are using tech-enabled innovations to make health and social care more accessible, efficient, and patient-centered. MIC is a program of Acumen America, funded by The Leona M. and Harry B. Helmsley Charitable Trust and the Public Benefit Innovation Fund at Digital Harbor Foundation. The Center for Health Care Strategies is a technical assistance partner to the collaborative.
- Improving Diabetes Outcomes Through Home Delivered Healthy Foods and Education: FarmboxRx and Molina Healthcare – To improve self-management of diabetes among Medicaid enrollees in Louisville, Kentucky, Passport by Molina Healthcare collaborated with FarmboxRx to design a pilot offering food delivery, health education, and telephonic engagement. - Partnering to Provide Digital-Driven Nutrition Services: Attane Health and Anthem Blue Cross Blue Shield– To combat food insecurity among Medicaid enrollees with diabetes in Kentucky’s Jefferson County, Anthem Blue Cross Blue Shield partnered with Attane Health to implement a pilot program offering home-delivered groceries and tele- and text-based nutrition coaching.
