Patient- and staff-reported measures are a key way to assess the quality and experience of care for people with complex health and social needs. The Center for Health Care Strategies worked with health care organizations to test new measures for understanding patient and staff experiences related to well-being, equity, goals of care, and care integration. The sites were participants in Advancing Integrated Models, a national initiative supported by the Robert Wood Johnson Foundation.

This brief shares insights on using these patient and staff measures from three health care organizations — the Center for the Urban Child and Healthy Family at Boston Medical Center, Hill Country Community Clinic, and Denver Health. Their experiences can inform other health systems and providers in more effectively assessing the impact of their complex care programs.
