States across the country are developing comprehensive, statewide multisector plans for aging (MPAs) to better coordinate policies, programs, and investments that affect older adults, people with disabilities, and their caregivers. An MPA is a state-led, cross-sector plan — typically spanning 10 or more years — that brings together partners across government, health care, social services, and other sectors to address aging-related needs in a more coordinated way.

The Multisector Plan for Aging Toolkit is designed to support states and their partners as they develop, implement, and periodically refresh an MPA. The toolkit offers practical guidance, concrete strategies, and real-world examples drawn from states that are actively engaged in this work. While the toolkit modules are presented as a sequence, states can use them individually based on where they are in the MPA process.

Toolkit Modules

Getting Started with an MPA – Introduces the concept of a multisector plan for aging and outlines the overall MPA development process. This module describes key phases of the work — from building the case to ongoing refinement — and explains how MPAs differ from other aging-related state plans.

Securing State Leadership Buy-In – Focuses on strategies for obtaining authorization and sustained support for an MPA, typically through an executive order or legislation. The module outlines considerations for navigating political and budgetary contexts and shares approaches for engaging leaders early and throughout the process.

Working Across State Agencies and Sectors – Provides guidance on establishing governance structures and cross-agency collaboration to support MPA development and implementation. This module covers strategies for forming steering committees and subcommittees, assigning leadership roles, and maintaining accountability.

Engaging Stakeholders and Community Members – Explores approaches and engagement methods for involving a wide range of partners in the MPA process, including older adults, people with disabilities, caregivers, advocates, providers, and partners outside aging and health sectors.

Creating a Data-Driven Approach – Details how states can use data to build support for an MPA, inform plan development, track implementation, and guide future updates. This module includes practical guidance on identifying relevant data sources, selecting indicators, and communicating progress and impact.

Drafting, Refining, and Refreshing an MPA – Outlines a structured approach for drafting the MPA document and establishing processes to update it over time. The module addresses roles and responsibilities for writing the plan, setting goals and initiatives, and refreshing the MPA as the landscape and priorities evolve.
