Today, few digital health products are developed to support individuals with complex physical, behavioral, and social needs. This database of digital health products, developed by the Center for Health Care Strategies through support from Kaiser Permanente Community Health, is intended to help health care providers, payers, and other stakeholders identify tools that are designed to improve care, engage patients, and address the health and social needs of complex, high-cost patients. While not exhaustive, the database provides an initial inventory of innovative digital health products for high-need, high-cost populations. This list will be updated periodically.
